Do you need PPTC 153 or a renewal?
If your most recent adult passport was issued in the last 15 years, at age 16 or older, for a 5- or 10-year term, and your details have not changed, you may be able to use the simpler renewal (PPTC 054) instead. If not, PPTC 153 is the right form. Not sure? Answer a few questions to check.
